Dr. Sikorski’s Top 10 for 2023

  1. Handheld ultrasound for non-invasive visualization of the skin and deeper structures

We can now visualize below the surface. Ultrasound enables us to distinguish between fat, skin, fillers, cysts, lipomas, vascular lesions, and bones. Different structures absorb or reflect the sound waves. Handheld ultrasound aids us in diagnosis and treatment. In addition, it provides a multi-layer picture of tissue layers. It is a view into the world below the surface, like a skin periscope.  

  1. Daxxify: New Botulinum Toxin

Daxxify is the newest neuromodulator for wrinkles. Daxxify follows Botox, Dysport, Xeomin, Jeuveau, and others. It is a longer-lasting toxin, in part because it has a proprietary positively charged stabilizing peptide. According to the manufacturer, the protein enables it to stay around the nerve terminal longer.

The product is made in the US and has a median duration of 6-months in the frown lines (glabella) and 5 months in the crow’s feet and forehead lines. It is a good option for those wanting the convenience of twice-a-year treatment. We are one of the first practices in the US to receive Daxxify for our patients. 

Newer insights into facial anatomy and newer dose studies have made all Botox treatment more precise.

  1. TXA for Melasma

Melasma is patchy areas of brown in the skin. Melasma can be difficult to treat, especially when it is deep in the skin (dermal rather than epidermal). Tranexamic acid, TXA, is a new treatment for melasma. It comes in pill form and in topical lotions. TXA reduces the deeper pigmentation. Although it is not a cure for this chronic condition, it is an effective treatment for melasma patients.

  1. Pico laser for brown spots on the face and body

As we age, sun-induced lentigos or brown spots start to appear on our face and body. The Pico laser is specific for brown spots. It is the best treatment for low contrast, light brown spots that devices like IPLs cannot improve. Pico laser can treat hard to resolve brown spots because of its selectivity for brown pigment.

  1. Neck – Radiesse filler for collagen stimulation and thickening of dermis

Our Radiesse-treated patients are thrilled with the improvement in their neck skin. The skin on the neck ages and thins faster than the skin on the face. Therefore, the neck skin often looks older than the facial skin. Radiesse is an easy filler treatment for improving the quality of the neck.

  1. Micro-coring – Newer treatment for tightening the neck, face, and mouth skin

Ellacor Micro-coring is technology that was invented at Massachusetts General Hospital. It is a non-laser, non-heat based device for treating skin laxity and wrinkles. It removes tiny micro-cores of skin without leaving a scar. It is based on the fact that removing many tiny bits of skin leaves no scar. It is a novel approach to improving skin laxity and tightening of the neck, mouth lines, lower face, and jawline.

This procedure is good for those who are not ready or willing to have a facelift. For those after they have had a facelift and want a refresh and those with wrinkles or laxity around the mouth.

  1. Vbeam and Fraxel lasers for early treatment and prevention of acne and acne scars

Studies are showing that early treatment of all scars reduce scars with fewer treatments. In acne, treatment with Vbeam, our vascular laser, and Fraxel, our low-down time remodeling laser, can reduce the development of depressed scars, as well as treatment visits. Early intervention is better for both prevention and treatment. The old adage “wait a year for scar treatment” has shown not to be the best practice for scar treatment.  

  1. Tattoo Trifecta Removal: Pico laser, PFD Patch, and Fractionated Lasers

Pico laser not only breaks up brown pigment; it also breaks up tattoo ink. Through acoustic mechanical energy that is 1 trillionths of a second, it reduces pigment into fine particles. When you add a PFD patch, you can do multiple treatments of the tattoo in one setting; adding a fractional laser helps extrude the ink out of the skin. All 3 – Voila – the most effective tattoo treatments!

  1. Resistant wrinkles – Phenol Chemical Peel

When all else fails for deep wrinkles, a deep peel may be the answer. Phenol chemical peels can be done in a segmental way to improve eyelid wrinkles and perioral (mouth) wrinkles. Phenol peels can improve lip lines and sun damage on the lips. Sun damage is common on the lower lip. This peel is best for lighter skin types.

  1. Treatment of Pre-cancerous lesions
  • We have switched our photodynamic therapy (PDT) protocol to be nearly painless. PDT is an in-office procedure that can treat an entire field – including the face, scalp, chest, or arms. This treats pre-cancerous lesions. We often combine the PDT treatment with an IPL. Together these treatments treat precancerous brown spots and broken capillaries.
  • Klysiri is a new once-a-day, 5-day topical treatment for pre-cancer.
  • We have many chemotherapy cream regimens that can be tailored to your needs.
  • Thulium laser is also used for actinic keratosis, skin texture, and pigment improvement. This laser is one of our favorites!
